Health Services
Through bilingual outreach, case management and community partnerships, ACLAMO helps link at-risk children, adults and seniors to health insurance, prenatal/pediatric care, primary and specialty care, dental care and mental health services. We also provide breast health education, screening, diagnostic and treatment services for hundreds of low income limited-English residents.
The overall goal of our program, which we call the Conexiones por Salud, is to improve health status and reduce health disparities for the target community of children and adults with limited English, and others at risk in the neighborhood surrounding ACLAMO’S offices. During the program year just concluded, Conexiones sought to address these three interrelated objectives:
1) Strengthen key community partnerships to bridge gaps of language and culture, especially with Pottstown School District’s PEAK program, Community Health and Dental Care, Inc., Pottstown Memorial Medical Center and Creative Health Services, and Fox Chase Cancer Center.
2) Provide bilingual outreach, community education and case management to link 150 children and adults to essential health resources. These resources include health insurance, maternal and child health services, primary and specialty care, dental care and mental health services; assistance for Spanish-speaking seniors with a range of nutrition, health and wellness needs; (health insurance and benefits, PA PACE, Medicare, connecting to physicians); linkage for young children and families to health screenings, vaccinations, dental care and other health services needed to register for PSD’s preschool, kindergarten and elementary school programs.
3) Provide a breast-health education and screening program for up to 300 Latina and other women at risk in Pottstown, bridging cultural and language barriers. During 2009-10 Conexiones had the goal of implementing Pottstown’s first bilingual breast health initiative, providing information and access to free screening for 100 women, as well as further diagnostic services, treatment and support for those in need.
